Finding help with health insurance, financial assistance, food resources and housing can be difficult.

The Community Health Navigation (CHN) program was created to assist our neighbors and community with finding needed resources and services in Tarrant County and surrounding areas.

We can also assist with other resources and services. 


CHN team members are certified site navigators through the Texas Health and Human Services and certified application counselors through the Centers for Medicare & Medicaid Services.

We serve as health insurance navigators and can assist clients with locating and enrolling in programs they need. This includes:

  • Affordable Care Act – Federally-Facilitated Marketplace
  • Medicare
  • Medicaid
  • Children’s Health Insurance Plan (CHIP)
  • Healthy Texas Women
  • JPS Connection
  • Baylor Scott & White Health Plan
  • Local health clinics

We also assist with the location and enrollment of social services, including:

  • TANF Cash Help
  • WIC Food Benefits
  • Financial assistance

The CHN team has connected more than 500 community members with social service programs and more than 300 community members with health resources, including health insurance and primary care physicians.
