About Chronic Disease Prevention

Though not contagious or infectious, chronic diseases inflict many people. These diseases can come on suddenly or develop over time. Some of these diseases may be incurable, but most – if not all – are avoidable. Tarrant County Public Health’s Chronic Disease Prevention division helps individuals learn more about these diseases so they can avoid them or better manage them.

Our Community Health Outreach has teams of trained professionals available to help educate individuals, organizations and communities on a number of chronic health issues as part of our goal of improving health equity across Tarrant County.
