...get a copy of Death Certificate?

Death certificates are filed/issued by the Local Registrar of the area that the death occurred in. In Tarrant County, the common practice of the incorporated cities is to file for their city exclusively. The records for unincorporated areas are all filed with the Tarrant County Clerk's office at 200 Taylor Street, 3rd Floor, Ft. Worth, TX 76102.

The person requesting the record must:

  • be an immediate family member
  • provide the full name of the deceased
  • provide the date of death
  • provide the city of death
  • provide the parent's names (if possible)
  • give a reason for requesting the record
  • provide a photocopy of THE REQUESTOR'S ID

The fee is $21 for the first copy and $4 for each additional copy requested at the same time. No searching is done without meeting these requirements as death records are confidential records in the State of Texas.

Both parties must be present at the County Clerk's Office.

...get forms for simple legal procedures, such as divorce or name changes?

For most legal procedures, including divorce and name changes, standardized forms do not exist. Documents must be created using sample wording obtained from legal form books.

The Tarrant County Law Library staff can direct you to the relevant information that you can use to select the proper paperwork. You can then photocopy the information and use it to create your own personalized documents. Be advised that the library staff cannot select the documents for you or advise you in drawing them up. It will be your responsibility to read the information and identify what is necessary for you own individual situation. In addition the library does not keep track of the various filing fees.

An application for a D.B.A. (Doing Business As) can be downloaded from the County Clerk's office via the internet listing for our Vital Statistics office or picked up in person at any of our office locations. The locations are:

Each applicant must appear in person in our office with valid picture identification or each applicant must appear before a notary public to have their signature acknowledged on the completed form before it can be accepted for filing. All parties wishing to list their names as owners on these certificates must present positive legal identification. Documents may be notarized in the County Clerk's Office or staff can acknowledge signatures.
