As part of our ongoing effort to safeguard the community’s health, Tarrant County Public Health maintains the Tarrant County Immunization Registry (TCIR). The TCIR is a confidential, automated system designed to track immunizations.
To request immunization records for yourself or your child, contact any of our Public Health Center locations, or call our Vaccine Distribution Center: 817-321-5450. There is an $8 charge per immunization record. Payment can be made online or by phone. Credit and debit card payments are charged a 2.85 percent, or a $1 minimum convenience fee, which is payable to Certified Payments, Inc. This will be shown as a seperate transaction on your credit/debit card statement and it will apply for each processed payment.
(The convenience fee is charged by the credit card processing
vendor. TCPH does not receive any portion of the fee. The rate is
solely determined by the vendor and is beyond the control and
authority of Tarrant County Public Health.)
You can request your record or your child's TCIR immunization record by email, by following these steps:
Download and print the Authorization to Release Official Immunization record (requires Adobe Acrobat Reader).
Fill out the request form clearly and completely.
Email the Authorization to Release Official Immunization Record to the email address noted on the form.
