    Adult Health Services will increase the percentage of neonatal/prenatal reactors issued by surveillance that are closed within seven days by five percent by July 31, 2015.

    By Sept. 30, 2015, the number of CDSMP=Health4Me graduates, who complete a 30-day post-graduation follow-up will be increased by five percent.

    Client Pre Registration Project

    Between Aug. 1 and Dec. 1, 2015, there will be a 10 percent increase in client preregistration for afternoon clinic times.

      Consumer Health

    Team Effort

    From April 1 to June 30, 2015, each month Consumer Health will have no more than 80 food establishment inspections past due by one year.  


    Notifiable Conditions Reporting

    By Sept. 1, 2015, we will increase the number of health care provider groups by five percent above the average number of reporting groups.

      Health Informatics

    Lowering the cost of illness in the community by reducing the burden of disease in the exposed population 

    By 2015, there will be a five percent reduction to the cost of illness in the target population through the utilization of disease intervention methods by the informatics team.

      HIV Administration

    Cultural Competency

    By Sept. 15, increase the percentage of Ryan White funded staff who complete an annual cultural competence self-assessment by 75 percent.

      North Texas Regional Laboratory

    Lab Submissions 101

    Increase participation in laboratory in-service training, pertaining to the submission of appropriate clinical specimens and requisitions, from the existing baseline of 57.7 percent for the Adult Health Services (AHS) staff to 90 percent by Sept. 30, 2015.

      Preventative Medicine Clinic

    Improving TB Testing Requirements and Results for the PMC.

    By Sept. 30, 2015, the PMC will show at least a 50 percent increase in patients completing their annual TB tests.

      Public Health Preparedness

    Answering The Call

    Public Health Emergency Preparedness will measure and improve the overall ICS Staff call down reponse rate to 80 percent and increase the overall availability rate to 70 percent by Sept. 30, 2015.

      Tuberculosis Elimination

    TB Meds- Video DOT with Rifapentine/Isoniazid (3HP)

    By Oct. 23, 2015, patients receiving 3HP via Video- Directly Observed Therapy (V-DOT) and beginning treatment June 1 to Aug. 7, 2015, will have an average number of missed doses decrease by one per person over baseline.

      Travel Health Services

    Don’t' Forget

    We will decrease the number of missed appointments from 20 percent of total appointments to 10 percent by the end of August 2015, in Travel Health immunization clients.


    WIC Call Center Hold Times

    By Aug. 31, 2015, WIC Call Center Time Frames over eight minutes will decrease from 56 percent to 20 percent.


    Reassessing Risk

    From July 1 through Sept. 30, 2015, all WIC clinics will complete 90 percent of high risk follow up phone calls within policy-dictated timeframe and 90 percent of monthly QA tools.
