    By November 1st, 2014, AHS staff's knowledge of clinic patient flow will increase by 20 percent.

    The number of client satisfaction surveys submitted for the Southside WIC Clinic will equal or exceed 10 percent of the number of clients seen in August 2014.

     Fiscal Services

     Daily Deposits

    Over a 90-day testing period ending on July 4, 2014, there will be a decrease in the number of missed daily divisional deposits after the Fiscal Services (Business Office) division expands its daily revenue deposit schedule period from 2-4 p.m.  to both 9-11 a.m. and 2-4 p.m.

     Chronic Disease Prevention

     Reach and Inform

    By September 30, 2014, 90 percent of the total staff in Tarrant County Public Health Environmental Division will know about all of the services offered by the Chronic Disease Prevention Division.

     Community Health Promotions

     Immunization Record Request

    Between August 1 and August 31, 2014, 10 percent of immunization records requests will be electronic.


     DSRIP Selection Policy

    Through the implementation of a DSRIP Policy and Procedure for accessing DSRIP funds, the DSRIP Division will award 75 percent of projects submitted, up to $50,000.

     Consumer Health

     Team Effort

    From June 1, 2014 to September 30, 2014, Consumer Health will see a 20 percent decrease in food establishment inspections that are past due by one year, over 2013 baseline data.

     Environmental Health

     EH /Application Tracking

    By November 30, 2014, average reduction of 1 incomplete OSSF permit per month for July and August.

     EH / Vector Control

     TCPH West Nile Virus Program Improvement

    After providing more in-depth training and education during the annual West Nile virus surveillance kick-off meeting given on March 20, 2014; general knowledge will be improved by 20 percent. 


     Epidemiology/Notifiable Conditions Reporting

    By October 24, 2014, we will increase the number of health care provider groups by 5 percent above the average number of reporting groups.

     Health Informatics

     Community Health Information Exchange

    By September 2014, there will be 75 additional cases of pertussis detected from John Peter Smith (JPS) Hospital through the health information exchange.

     Health Planning & Policy

     Health Planning & Policy 101

    By September 30, 2014, there will be a 35 percent increase of Staff grades 72 and above familiar with HP & P Services.

     North Texas Regional Laboratory

     Lab/Extension of the Coliform Sample Submission Cut-off Time

    By June 30, 2014, we will increase our water bacteriology customer service rating of "Excellent" by 20 percent regarding "Hours of Operation".

     Preventative Medicine Clinic

     PMC/Program Revenue

    By September 30, 2014, the PMC will increase revenue from copays and fees 25 percent above current revenue.

     Public Health Preparedness

     Increasing Public Health Safety Warden's Knowledge

    Public Health Preparedness will measure and improve the overall average safety knowledge score of Public Health Safety Wardens by 15 percent by October 15, 2014.

     Tuberculosis Elimination

     TB/TB Meds

    By September 30, 2014, 10 percent more people will complete Latent TB Infection (LTBI) treatment when given a combination of Rifapentine/Isoniazid (3HP) over the 2007-2011 treatment completion rate.

     Travel Health Services

     Customer Form Simplification

    The THS form completion times will be reduced by at least 25 percent before August 1, 2014.

     Community Health Promotion/WIC

     WIC Breastfeeding Increase Exclusive Breastfeeding Infants

    By changing participant flow targeting new La Gran Plaza WIC mothers we expect to see an increase in exclusive breastfeeding infants between January 1, 2014, and September 30, 2014.


     CHP/WIC Call Center Hold Times

    By August 31, 2014, WIC call center hold times will be within 8 minutes or less 80 percent of the time.


     WIC/Risking It All

    For August and September 2014, all WIC clinics will complete 90 percent of high risk follow-up phone calls within the appropriate time frame and 100 percent of high risk quality assurance tools.
