    HOW DO I BECOME A WIC CLIENT?

    • Call 817-321-5400 to schedule a WIC appointment at your local WIC office.
    • To find your local WIC office, visit the WIC Clinic locations page or Texas WIC locator.
    • You must apply in person at your local WIC office and bring the child who is applying.
    • Complete (front and back) and bring the WIC-35-3, WIC Income Questionnaire, WIC-35, Family Certification form, to your scheduled WIC appointment. The WIC Income Questionnaire will assist you in determining what income sources to bring to your WIC appointment.
    • Bring the following documents to your WIC appointment:

     

    1)   Identification for both parent/guardian applying AND for child applying for WIC, such as:

    • Birth certificate
    • Driver's license/ID card (school, employee or official)
    • Social Security card
    • Immunization record
    • Military ID
    • Medicaid, Food Stamps or TANF letter/form (applicant must be identified as a recipient of benefits)
    • Foster placement letter
    • WIC Lone Star card, if you already receive WIC
    • Passport
    • Baby’s ID can be: Crib card, Immunization Record or Baby Hospital ID bracelet

     

    2)   Proof of your current address, with your name and complete address with zip code on it, such as:

    • Utility Bill (water, electric or phone)
    • Lease agreement or letter from your landlord
    • A business letter
    • A signed letter from a person you are living with and their proof of address with their name on it.
    • A voter’s registration card
    • Letter from a homeless shelter
    • Medicaid, Food Stamps or TANF letter/form
    • Foster placement letter
    • Or you can complete and bring in WIC-R02, Residency form (Must be accompanied with proof of address and completed by utility bill holder)

    3)   Proof of income for everyone in the same household, such as:

    • All current income for the last 30 days 
    • Child support statement (can be printed from Child Support website, www.childsupport.oag.state.tx.us) or by calling 1-800-252-8014.
    • Social Security benefits statement (to retain a copy of award letter, call 1-800-772-1213)
    • Unemployment statement (debit card issued cannot be used as proof of unemployment income)
    • Signed statement from employer, with employer name and contact phone number
    • WIC-19b, Employment verification form, must be completed by your employer and signed by actual employee to be released
    • WIC-19a, Assistance documentation form (if your household has NO income but receives assistance from family, friends or an organization)      (THEY CANNOT LIVE IN THE SAME HOUSEHOLD!!!)
    • WIC-32, Applicants statement of Farm/Self Employed
    • If you currently receive Medicaid, Food Stamps or TANF, you will need to bring the current copy of your eligibility forms

     

    4)   Immunization Records for infants and children, if available.
